What’s changing

Beginning May 4, some behind-the-scenes systems, like patient records, billing and communication tools, will transition to Sanford Health. 

Locations, phone numbers, providers and care teams remain the same, except for Rapid City Medical Center, which has updated location information.

Frequently asked questions

What is changing on May 4?

On May 4, Black Hills locations, Creekside Medical Clinic and some providers from Rapid City Medical Center will transition to Sanford Health systems for electronic medical records (EMR), patient communications, billing and scheduling. Your care team and services will remain the same, but you’ll start seeing “Sanford Health” in messages, portals and statements.

Will my doctor or care team change?

Your provider and care team will stay the same if they are at a Black Hills locations or Creekside Medical Center. 

Your care team may change if you see a provider at Rapid City Medical Center, as they will be practicing at a new location. In other words, you’ll have the same provider but new care team members.

What happens to my medical record?

Your medical record will move into Sanford Health’s My Sanford Chart if you are a patient of: 

  • Black Hills Orthopedic & Spine Center 
  • Creekside Medical Clinic 
  • Black Hills Surgical Hospital 
  • Black Hills Urgent Care 

This allows your care team to easily access your information for a more connected care experience. 

Some of your previous health information may not appear as we transition, but any information from visits on or after May 4 will populate in My Sanford Chart.

Your medical record will stay in eClinicalWorks and the healow app if you are a patient of a provider now at Sanford Health Jordan Drive Clinic, Sanford Health Tower Road Clinic or The Endoscopy Center, LLC, (all formerly providers at Rapid City Medical Center).

What patient portal and app should I use?

Patients of Black Hills Plastic Surgery

Please begin using the Symplast app and patient portal to manage your care at Sanford Health Fifth & Texas Clinic, the new home of Sanford Health plastic surgery services.

Patients seeing a Rapid City Medical Center provider now at Sanford Health Jordan Drive Clinic, Sanford Health Tower Road Clinic or The Endoscopy Center, LLC

Before May 5: Continue using your current patient portal and app for appointments and records related to visits on or before May 4.
Starting May 5: You will continue to use the healow app to manage your care.

Before May 4: Continue using your current patient portal for appointments and records related to visits on or before May 3.
Starting May 4: You will use My Sanford Chart or the Sanford app to manage your care.

With My Sanford Chart and the Sanford app, you can:

  • Message your care team
  • View test results
  • Manage and schedule appointments
  • Complete check-in forms
  • Pay bills

Will I receive messages from Sanford Health?

Yes. Beginning May 4, appointment reminders, visit updates, billing notices and other communications will come from Sanford Health through phone, text, email or mail.

For patients of Black Hills Orthopedic & Spine Center, Creekside Medical Clinic, Black Hills Surgical Hospital and Black Hills Urgent Care:

To enroll in text messaging with Sanford Health, text “START” to 23701. Through texting you can:

  • Pre-register for your visit
  • Complete forms
  • Update your health info
  • Get appointment reminders
 

Where do I pay my bill?

This may vary depending on dates of service. You may temporarily see bills from more than one system.


Rapid City Medical Center
  • Services on or after May 5: Bills will come from Sanford Health.
  • Services on or before May 4: Bills will continue through Rapid City Medical Center billing systems. You can pay these bills using the Rapid City Medical Center payment portal.

All Other Patients

Paying Sanford Health bills for services after May 4

For patients of Black Hills Orthopedic & Spine Center, Creekside Medical Clinic, Black Hills Surgical Hospital and Black Hills Urgent Care:
For services received on or after May 4, you’ll have multiple ways to pay Sanford Health bills, including online, via My Sanford Chart and the Sanford app, by phone, by mail or in person.

How do I schedule an appointment after May 4?

You can continue to schedule in many of the same ways or through Sanford Health.

For patients of Black Hills Orthopedic & Spine Center, Creekside Medical Clinic, Black Hills Surgical Hospital and Black Hills Urgent Care:

  • Schedule online through My Sanford Chart, the Sanford app, or here on the website.
  • Schedule through phone by contacting your clinic directly

For patients getting urgent care at the Haines Ave Clinic and Baken Park Clinic, scheduling and “Reserve My Spot” options continue to be available.

 

Can I still visit the same locations?

All Black Hills clinic and hospital locations remain open and continue to serve patients.

Sanford Health providers will not be located at the Rapid City Medical Center. You can find new location information for providers who worked out of the medical center, now located at the Jordan Drive Clinic and the Tower Road Clinic.

How will I get my prescription refills?

When you need a prescription refill, please continue to contact your pharmacy. If you're out of refills and need a prescription renewal, contact your provider. Once you’ve established care in My Sanford Chart, you will also be able to request renewals in the app.
