Get your child ready for sports and other school activities during our sports physical event at the Sanford Children’s Campus, building A, in Bismarck. A sports physical visit ensures your child can safely participate all year long.
This low-cost visit includes:
- A physical exam with heart and lung checks
- Review of injury and concussion history
- A check-in about lifestyle and habits
- Certain immunizations as needed (billed through insurance)

- Patient Registration Form
- Statement of Financial Responsibility
- Acknowledgment of Notice of Privacy Practices
Note: Additional services provided during your visit that are not listed above will be billed separately to your insurance. If you prefer to file insurance for your appointment, please schedule a well-child visit with your primary care provider. This comprehensive visit includes sports physical requirements and can be billed to your insurance with no payment necessary at the time of your appointment.
