Get your child ready for sports and other school activities during our sports physical event at Sanford Sports in Bismarck. This event is for Bismarck High School athletes who are members at Sanford Sports or are partaking in summer training. A sports physical visit ensures your child can safely participate all year long.

This low-cost visit includes:

• A physical exam with heart and lung checks
• Review of injury and concussion history
• A check-in about lifestyle and habits
• Certain immunizations as needed (billed through insurance)

Appointments are encouraged, but not required. Call (701) 323-1125 to schedule a $35 sports physical at this event.

• Signed Sports Physical Form
• Patient Registration Form
• Statement of Financial Responsibility
• Acknowledgment of Notice of Privacy Practices
