Making our communities stronger and healthier is at the core of everything we do at Sanford Health. As the nation’s largest non-profit rural health care system, we’re committed to delivering world-class care no matter where you live.

And that care extends beyond our medical centers and clinics. We’re active in the communities we serve, and we invest substantial resources into improving access to affordable, high-quality care, enhancing public health and addressing health disparities—all while making our communities more resilient.

Community Commitment Report

Sanford Health publishes an annual Community Commitment report to share key programs and services that have been implemented across our footprint to meet the needs of our communities and improve the overall health and wellbeing of those we serve.

Identifying community needs

Health conducts the Community Health Needs Assessment to learn more about the health and well-being of everyone we serve. The Community Health Needs Assessment allows us to identify significant unmet health needs and prioritize local partnerships and initiatives.

Your participation in the Community Health Needs Assessment helps improve care in your community. Please complete the survey when it opens again in October 2026.

Investments that make a difference

Every community is unique, so we consider local conditions for each funding request. We have established criteria to guide these decisions.

Charitable donations, sponsorships and community investments
Sanford Health regional offices review funding requests as they are received. Funding requests must demonstrate an identified community health need. Initiatives that support community health priorities may take precedence.

Please allow up to six weeks for your application to be reviewed. Additional processing time may be necessary if you are a new vendor or additional information is needed to process payment.

Please note that Sanford Health can’t accept funding requests from:

• For-profit organizations
• Political campaigns and candidates
• Individuals requesting personal funding
